Activation Code
The service plan activation code is located on the activation code email (or your Service Plan Activation Summary if you have Epson Partner Portal access).

Hardware Serial Number
The hardware serial number is located on the side, back, or bottom of your Epson product.

This field must contain numbers and letters only, serial number does not include the * (asterisk) characters, if present.

Hardware Purchase Date
The hardware (or product) purchase date is used to determine service plan eligibility.

Hardware Place of Purchase
Please provide the name or location where your hardware (or product) was purchased.

ZIP/Postal Code Installation
Please provide the ZIP or Postal code where the hardware (or product) was installed.
